  • You must be at least 18 years of age to individually use our services. Individuals below 18 years of age who wish to use our services must be signed up and accompanied by a parent or guardian.

This Privacy Policy describes how we collect, use, and protect the personal information that you provide to us when you complete a health and wellness intake and schedule an online or in-person appointment.

Information We Collect

We will not collect, share, or store any personal information that is not necessary to provide you with services that you have requested. We collect information needed to assess your health and wellness, design customized plans for improvement, schedule and confirm appointments, provide you with requested health and wellness services, communicate about your appointment, process payments, and improve service quality. 

How We Protect Your Information

Due to the sensitive nature of health and wellness information, we take all reasonable steps to protect your privacy and personal information from unauthorized access, use, or disclosure using industry-standard security measures including SSL encryption, firewalls, and end-to-end encryption for all video and audio communications during your appointment. We also have policies and procedures in place to train our staff on how to protect your privacy.

Sharing Your Information

We will not sell or rent your personal information to any third parties. We will only share your personal information with third parties who have a legitimate need to access your information in order to provide you with services such as other integrative health practitioners who may consult to provide enhanced services under defined circumstances. 

You can choose not to provide us with your personal information however this will restrict our ability to provide requested services. You can request access to your personal information, have it corrected, or deleted upon request. Please direct questions to info@iqperformancehealth.com.

CANCELLATION

We understand that things sometimes happen outside our control. Please note we need at least 24 hours' notice for any cancellations. Cancellation requests within 24 hours may be subject to a non-refundable cancellation fee. 
 
If you miss your appointment without any prior notice, you will be considered a no-show and charged the full appointment fee. To cancel or reschedule your appointment, please email us or use our online scheduling system.

This Consumer Health Data Privacy Policy ("Policy") applies to IQ Performance Health LLC ("we," "us," or "our") and describes how we collect, use, disclose, and otherwise process your Consumer Health Data ("CHD") in compliance with the Washington My Health, My Data Act ("MHMDA").

CHD is broadly defined under the MHMDA and includes any information that:
 

  • Relates to your physical or mental health condition.

  • Identifies a consumer seeking healthcare services (including integrative wellness services).

  • Concerns your bodily functions or biometric information.

  • Can be used to infer information about your health or condition.

  • Is derived from non-health data that can be linked to you.

How We Collect CHD

We collect CHD from you in various ways, including:
 

  • Information you provide on intake forms, health questionnaires, and during consultations.

  • Data collected through wearable devices or apps you authorize us to access (with your separate opt-in consent).

  • Information from healthcare providers or other third parties, with your authorization.

  • Observations made during your sessions with us.

Your Rights Regarding Your CHD

Under the MHMDA, you have certain rights regarding your CHD, including:
 

  • Right to Access: You have the right to access your CHD in a readily usable format free of charge. We will provide you with a copy of your CHD electronically or by mail upon request.

  • Right to Correct: You have the right to request that we correct any inaccurate or incomplete CHD in our records.

  • Right to Deletion: You have the right to request that we delete your CHD, subject to certain exceptions.

  • Right to Opt-Out of Sharing: You have the right to opt-out of the sharing of your CHD with third parties, except for those we share with to provide you with services or as required by law.

Your Consent

We will obtain your informed, opt-in consent for the collection and use of your CHD before we collect or use it for any purpose beyond providing the requested services. We will obtain your separate opt-in consent for any "sharing" of your CHD with third parties, including any sharing with our affiliates. We will never require you to consent to the collection or sharing of your CHD as a condition of receiving services.

Your Consent to Sell is Not Required

We will not sell your CHD without your express written authorization.

Data Security

We take reasonable steps to protect your CHD from unauthorized access, disclosure, alteration, or destruction. These measures include administrative, physical, and technical safeguards.

Changes to this Policy

We may update this Policy from time to time. We will post any changes on our website and notify you of any material changes.

Geofencing Prohibition

We will not use geofencing around health facilities to identify or track you, collect your CHD, or send you targeted communications based on your CHD or use of our services.

Non-Discrimination

We will not discriminate against you for exercising any of your rights under the MHMDA.
